Scheduling your test
How to Schedule an Exam
- All types of examinations need to be scheduled in
advance. See the table above for the number to call.
To
streamline the registration process and minimize your
personal expenses, it is important to take the following
steps when calling for your appointment:
Step
1:
Find out
before hand if your policy or plan requires a preauthorization
or referral. If so, contact your insurance company and find
out what you need to do to obtain one.
Step 2: Give Associated Radiologists, PA
complete insurance information, including insurance card, name
and address of your plan or insurer(s), name and address of
your claims payer (if different from insurer), your ID and
group numbers, and the name of the policyholder or subscriber.
Things to
remember when scheduling your exam:
- Notify your physician and the Associated
Radiologists staff, if there is any chance that you
may be pregnant or are currently breastfeeding.
- Advise us of special circumstances such as
physical limitations, significant medical
conditions, and allergies.

Advise the Associated
Radiologists staff of your previous CT, MRI, PET
scans, ultrasounds or x-rays and where they were
performed.
- A cancellation notice of at least 24 hours is
required due to the need to accommodate other
patients.

Prescriptions
Your doctor has provided you with an order for your test.
This prescription contains valuable information for us
pertaining to your test. It
is essential that you bring the 'script' with you on the day of
your test. If you forget it, you may not be able to have
your test performed as scheduled. Please remember to bring
it with you.

Test Results
We recognize the importance of timely
results reporting. In most cases, we will have examination
results on their way to your clinician within 24-48 hours
Depending on the the type of test you have performed, it
takes anywhere from a few moments to several hours or even a
couple of days for our office to send results by fax or
telephone or mail to your referring doctor's office.

Referrals
Many of the managed care health insurance plans require you
to obtain a referral before visiting a specialist. Other plans
require no referrals. Each plan is different.
You should take it upon yourself to become educated about
YOUR health insurance. You enter into the terms of your
insurance contract with your insurance provider, NOT your
physician or Associated Radiologists. It is your responsibility
to make sure you understand the terms of your coverage and the
necessity - or lack thereof - of obtained referrals and what
services are allowed or not allowed.
Your insurance carrier has entered into contracts with
certain diagnostic centers for services. This may limit your
options as to what lab or imaging center you can use. It is
important for you to know who participates with your plan.
We are happy to assist you with your insurance, but cannot
accept responsibility for knowing all of the details of your
coverage. Please contact the customer relations department of
your carrier. Typically, the number is listed on your card. They
are far better prepared to answer questions regarding your
coverage than our office staff.
IF YOU HAVE AN INSURANCE THAT REQUIRES A REFERRAL... it may
be beneficial for you to request one as soon as you have
scheduled your appointment with us. Some primary care offices
take two weeks to process them. If you do not have a referral,
there is the possibility that you will not have your test done.
Precertification
Most insurers have a precert
program which is designed to ensure that services provided to
the patients are Medically Necessary, provided in the
appropriate setting, and covered under the Plan. The
patient (or someone acting on the patient's behalf, such as a
family member or the patient's Physician) must contact the
insurer at least 2 days before incurring any expenses for these
services or supplies.
Precertification requirements vary, depending on the benefit
plan. Please contact the customer relations department of your
carrier. Typically, the number is listed on your card. They are
far better prepared to answer questions regarding your coverage
than our office staff.
A Precertification number will be assigned to all approved
services.
All referral or precert authorizations
should be made out to :
“ASSOCIATED RADIOLOGISTS - (Center Name)”

Co-Payments
As a convenience to you the patient, we file
all claims directly with the insurance company and will assist
you in obtaining the maximum fair payment for a given
procedure. Associated Radiologists, PA is a Medicare
approved facility and the physician staff accept assignment on
all Medicare claims. Any balance not paid by Medicare is
forwarded to the secondary insurance company, the balance is
generally paid in full. Associated Radiologists, PA and
physician provider staff also participate in most local managed
care plans accepting payment in full for covered services of
those plans which do not have co-insurance or deductible
requirements.
The patient is ultimately responsible for any
known co-payment or deductible not satisfied at the time of
service, as well as non-covered services. In order to assist
us in providing quality services at a reduced cost, you will be
asked to pay any deductible or co-payment amount at the time of
service. Once payment is received by the insurance company,
any unpaid balance is forwarded to you. In the event that the
insurance company does not make payment within 45 days, the
unpaid balance becomes your responsibility and we expect payment
upon receipt of the final notice. Should either you or our
billing staff feel that the insurance company has been remiss in
payment for any given procedure, we will assist you in any way
possible to obtain payment for that expense. In many cases a
telephone call from you or our billing staff will be effective.
In other cases a specific letter indicating the medical
necessity for the procedure may have to be submitted by your
physician.

Punctuality
Due to the variable volume of imaging
needs, we are busier some days than others. We will try to
complete your exam in the shortest time possible, while
providing it in a caring manner.
We pride ourselves on staying on
schedule, which in turn will keep you on schedule. Striving to stay "on time" benefits us all. It decreases
stress and makes us all more efficient. We ask that you arrive
20 minutes prior to your office visit to allow enough time to
complete your registration. If you are going to be late for your
scheduled appointment, please call us to let us know.

Inclement Weather
A decision to close the office is not taken lightly. It is
made at the highest levels, taking into consideration the safety
of our patients and our staff.
We do not follow the local school closings. You will receive
a telephone call from one of our staff members advising you that
we have elected to close for the day. If you are unsure of the
status of your appointment and have not heard from us, please
feel free to contact the office.
Weigh the necessity of your visit against the risk of the
trip. Your well-being comes first, and we will be happy to
reschedule your appointment for a later date. If you do plan to
venture out, please use extreme caution, especially in the
parking lots and sidewalks. If we are open, and you don't call,
we can only assume you are planning to keep your appointment.

- Christmas Day
We are also closed half-days on the following:
- New Year's Eve
- Christmas Eve
- Good Friday

Financial Information
To review your financial statement or any other financial
matter pertaining to our office, please call and talk to one of
our competent, courteous Billing Department personnel at 732-968-1500 from 8am to 7pm,
Monday thru Friday..
We accept Visa and MasterCard, check, or cash as forms of
payment for co-payments and office fees.

presented to our staff at the time of your visit, we may have
to reschedule your visit to allow you to bring the information
to us.

Obtaining Copies of your test
If you need your films, please call the
Center where you had the test done to request that the films be
released to you. Please keep in mind that we require 72
- 96 hours advance notice to get the films from storage.
We will ask you to sign a release when you pick up the films
. If you only need a copy of the
report, it can be done within a short time. You will be
asked to sign a release.
